FLEXI Health Protect Gold & Silver FAQs

Health protect is a benefit plan which pays fixed amount for each 24 Hours spent in a hospital.


Hospitalization due to injury or illness or both is covered.


Yes, one day daily allowance is payable if treatment is carried out in day care center.


Daily Allowance is paid for each completed 24 Hours.


Yes, two times of daily allowance is paid for each completed 24 Hours that Insured Beneficiary spends in ICU.


No original bills are required only copy need to be submitted for claiming daily allowance.

Annual Preventive health check-up can be utilized by accessing the Service Provider app and clicking on the “Preventive Health Check-Up” tab.

Alternatively, you can call our customer service at 020-48562555 to get the voucher code and share it with the hospital / Lab.


Doctor’s prescription is not mandatory for availing these services.


All Wellness coverage (HPR) can be claimed/ utilised through “ Caringly Yours” Bajaj Allianz General Insurance App. It can be downloaded through Playstore.

 

Terms and Conditions with Exclusions

For credit card customers – Customer shall mean and include holder of a valid DBS credit card

  1. You acknowledge that the insurance product is underwritten by Bajaj Allianz General Insurance Company Limited (“the Insurer”).
  2. You acknowledge that DBS Bank India Limited (‘DBS’ / ‘Bank’) is corporate agent of Bajaj Allianz General Insurance Company Limited having license number CA0257.
  3. The policy covers the Customer and/or Customer’s dependents (as may be approved by the Insurer) to the benefits of the Policy as per the terms and conditions of the respective Policy. You confirm to have read and accepted the policy details available at <Webpage link of the product details>
  4. You recognize and understand that the details you submit as part of the application, will be considered by the Insurer for accepting your application for the insurance product. Any incorrect or false information by you may lead to rejection of your claim, if any.
  5. The policy coverage will start after issuance of Policy certificate by the Insurer.
  6. Master policy is the contract between contract between Bajaj Allianz General Insurance Company Ltd. and the Company Your participation in the policy is voluntary.
  7. DBS is a master policy holder for the Insurance policy. Credit card customers will receive a certificate of insurance on purchasing this policy
  8. You hereby authorize DBS to share your application including your personal and other information with the Insurer, in order to facilitate issuance of the policy.
  9. The individuals holding credit card shall be the applicant in all certificate of insurance issued by the Insurer for the insurance coverage applied through DBS Bank India Credit Card app
  10. You agree and acknowledge that if the credit card is closed for any reason or the card has been blocked, the policy cannot be renewed. The policy will stand discontinued from next renewal date.
  11. DBS shall not be responsible for any rejection or acceptance of application by the Insurer.
  12. You agree that DBS is a corporate agent of Bajaj Allianz General Insurance Company Ltd. and holds out no warranty or makes no representation about quality, delivery of the policy or claims processing whatsoever by the Insurer.
  13. You agree to review the Policy document received from the Insurer after acceptance of your application. You shall be bound by all the terms & conditions of the Insurer relating to the grant of insurance that are in force and as amended from time to time.
  14. Claims Processing: The insured person/beneficiary shall get in touch with the Insurer directly, in case of claims and shall not involve DBS in any manner whatsoever. The insured person/legal heir shall undertake to intimate the Insurer, fill up the claim form and support claims with appropriate documents as per the normal procedure laid down by the Insurer.
  15. Cancellation: You may at any time cancel the policy in full as per the terms and conditions of the relevant policy and in such event the Insurer shall allow the refund of premium, if any, at short period scales only as indicated in the policy document, provided no claim has occurred up to the date of cancellation. This grid may be reviewed by the Insurance Company from time to time and changes would be communicated in the cover note or through an alternate communication by the Insurance Company.
  16. You agree that you have read and understood the product details which include product exclusions, benefits and coverage.
  17. On accepting the terms and conditions, you will be redirected to third-party website owned and controlled by Bajaj Allianz General Insurance Company Ltd. Your use is governed by the terms of third-party website and DBS  is not liable for such use
  18. The renewal of this policy is subject to Group Policy being renewed with the DBS as master policy holder.
  19. The premium amount (along with applicable taxes) will be debited on your credit card and billed in your immediate statement. Please ensure sufficient limit is there on the card.
  20. You can opt for SI to enable seamless renewal payments in the payment gateway. However, in case of any change in card num, the SI will not be executed, and a fresh registration will be required to debit the premium.
  21. You will not earn any reward points for the premium amount.
  22. In case of voluntary or involuntary card closure, the policy will stay active only for the current year (further renewals will not happen).

Exclusions:

General Exclusions

  1. Any dental treatment that comprises of cosmetic surgery, dentures, dental prosthesis, dental implants, orthodontics, surgery of any kind unless as a result of Injury to natural teeth and also requiring Hospitalisation.
  2. Medical Expenses where Inpatient care is not warranted and does not require supervision of qualified nursing staff and qualified medical practitioner round the clock
  3. War, invasion, acts of foreign enemies, hostilities (whether war be declared or not), civil war, commotion, unrest, rebellion, revolution, insurrection, military or usurped power or confiscation or nationalization or requisition of or damage by or under the order of any government or public local authority.
  4. Any Medical Expenses incurred due to Act of Terrorism will be covered under the Certificate of Insurance.
  5. The cost of spectacles, contact lenses, hearing aids the cost of crutches, dentures, artificial teeth and all other external appliances and/or devices whether for diagnosis or treatment except for Cost of  Artificial   Limbs,   Cost of prosthetic devices and other devices or equipment if implanted internally like pacemaker during a surgical process etc.
  6. External medical equipment of any kind used at home as post Hospitalisation care including cost of instrument used in the treatment of Sleep Apnoea Syndrome (C.P.A.P), Continuous Peritoneal Ambulatory Dialysis (C.P.A.D) and Oxygen concentrator for Bronchial Asthmatic condition.
  7. Congenital external diseases or defects or anomalies, growth hormone therapy, stem cell implantation or surgery except for Hematopoietic stem cells for bone marrow transplant for haematological conditions.
  8. Intentional self-Injury (including but not limited to the use or misuse of any intoxicating drugs or alcohol).
  9. Vaccination or inoculation unless forming a part of post bite treatment or if medically necessary and forming a part of treatment recommended by the treating Medical Practitioner.
  10. All non-medical Items as per Annexure II.
  11. Any treatment received outside India is not covered under this Certificate of Insurance.
  12. Circumcision unless required for the treatment of Illness or Accidental bodily Injury,
  13. Treatment for any other system other than modern medicine (allopathy)
